We performed an internal audit over a regulated entity’s investigation process and code of conduct-related controls, which included identifying control/process gaps and areas for improvement. The review also included making recommendations for enhancing the entity's whistle-blower hotline intake, triage, investigation, and quality-control/follow-up processes.
